Health Raiser Submission Form

Each submission will be read and considered for the NKU Health Raiser Award and the $20 gift certificate by the NKU Wellness Team. The winner will be announced on the Final Monday of each month. By submitting your entry, you agree, that should you win, you will allow yourself to be photographed and your story included in NKU Wellness publications and on the Wellness Web site.
